Required Forms for Admissions

The student’s file contains the following forms required by The Barbara Gordon Montessori School, AMI Teachers of Texas (AMITOT), Almost Home and the Texas Department of Family and Protective Services.  The forms must be completed by a parent or legal guardian and must be returned by the required dates.  Students will not be admitted unless all required forms are on file in the school office.

Application for Admission
This form is completed as the initial request for enrollment. Parents need to keep the information updated by notifying the office of new information.

Enrollment Contract
The contract describes the agreement between the parent and The Barbara Gordon Montessori School for enrollment of a child for the school year.

Health Record
Students enrolling in The Barbara Gordon Montessori School must have a current immunization record on file in the office.  New students must have a physical signed by a physician before admittance to school.

BGMS Authorization Form - This form provides the following:

  • Emergency contact information (should be kept current) - a new BGMS Authorization Form must be completed each year.
  • Carpool/Pickup Authorization - this lists those individuals who are authorized to transport the child to and from school.  You may make changes to this form by coming into the office.  A new Carpool/Pickup Authorization form must be completed each year.  No child will be released to an unauthorized person.
  • Field Trip Permission - authorizes a child to participate in scheduled off-campus trips.
